The information on this form will assist in either the provision of Adaptations to your current accommodation or the provision of suitable accommodation to meet your disability needs. This form will be passed to Charnwood Neighbourhood Housing's Specialist Occupational Therapist & in some cases this information may be forwarded to Social Services for an assessment. You will be informed if this is the case.
